ENTRY

  • Customers shop at any participating KPJ Healthshoppe outlet during the campaign period.
  • Customers are entitled to ONE (1) entry for every minimum spend of RM100 in a single receipt.
  • Customers must submit their entry by scanning the QR code and completing the Google Form with the following details:
    (Name, Contact Number ,Address, Email Address, Receipt attachment ,Purchase amount)
  • Eligible receipts may be accumulated throughout the campaign period.
  • More Spend, More Chances to Win: Every RM100 spent equals one (1) entry, increasing the customer’s chances of winning.
  • Winner announcement will be conducted within one (1) month after the campaign period ends.

WINNER SELECTION & ANNOUNCEMENT

  • Winners will be selected through a random lucky draw from all eligible online and offline entries.
  • The winner selection process will be conducted within thirty (30) working days after the campaign ends.
  • Winners will be notified via phone call and/or email based on the contact details provided during participation.
  • Winner’s names may also be announced on KPJ Healthshoppe official platforms (including social media and website), subject to the winners’ consent.

PRIZE CLAIM & REDEMPTION PROCESS

  • Winners must respond and confirm acceptance of the prize within seven (7) calendar days from the date of notification.
  • Winners may be required to provide proof of identity and original purchase receipt/order confirmation for verification purposes.
  • Prizes must be redeemed within the stipulated redemption period communicated by KPJ Healthshoppe upon confirmation.

FORFEITURE & REPLACEMENT WINNER

  • Failure to respond within the stipulated seven (7) days, failure to provide required verification documents, or failure
    to redeem the prize within the given timeframe will result in automatic forfeiture of the prize.
  • In the event of forfeiture, KPJ Healthshoppe reserves the right to select a replacement
    winner at its sole discretion, without prior notice.

GENERAL TERMS & CONDITIONS

  1. This campaign is open to all customers aged 18 years and above at the time of participation.

2. The campaign is valid for purchases made at participating KPJ Healthshoppe outlets and official KPJ Healthshoppe e-commerce platforms only.

3. Each eligible receipt or online order with a minimum spend of RM100 in a single transaction is entitled to ONE (1) entry.

4. Multiple entries are allowed with multiple eligible receipts or orders during the campaign period.

5. Employees of KPJ Healthshoppe Sdn Bhd and immediate family members (spouse, parents, children, siblings) are not eligible to participate.

6. Participants must ensure that all submitted information (including name, contact number, email address, and receipt/order details) is true, accurate, and complete. KPJ Healthshoppe reserves the right to disqualify any entry with incomplete, inaccurate, or fraudulent information.

7. KPJ Healthshoppe reserves the right to verify all receipts, order records, and participant details, and to reject entries that are illegible, duplicated, altered, tampered with, or suspected of fraud.

8. Winners will be selected via a random lucky draw from all eligible entries after the campaign period and will be contacted via phone and/or email.

9. Winners must respond within seven (7) calendar days from the date of notification to claim their prize. Failure to respond within the stipulated timeframe will result in forfeiture, and KPJ Healthshoppe reserves the right to select an alternative winner.

10. All prizes are non-transferable, non-exchangeable, and non-refundable, and cannot be redeemed for cash or other items.

11. KPJ Healthshoppe reserves the right to substitute any prize with another prize of similar value without prior notice should the original prize become unavailable.

12. KPJ Healthshoppe reserves the right to extend, shorten, suspend, cancel, or modify the campaign, mechanics, prizes, or these Terms & Conditions at any time without prior notice, subject to applicable laws.

13. By participating in the campaign, participants agree to allow KPJ Healthshoppe to use their name, photograph, and/or likeness for publicity, advertising, and promotional purposes without additional compensation, unless otherwise prohibited by law.

14. KPJ Healthshoppe shall not be held liable for any loss, damage, or injury incurred as a result of participation in the campaign or acceptance/use of the prizes.

15. Any disputes, claims, or matters arising out of or in connection with this campaign shall be governed by Malaysian law, and KPJ Healthshoppe decision shall be final, conclusive, and binding.
